EPI Primary Care Package

Over the past year EPION’s Community of Interest (CoI) has been working on a few documents to support primary care in the early detection and facilitation of access to our EPI services. We have created a generic letter and information sheet.

The idea has been that once a client has been referred to an EPI service, the EPI program would then send an information package back to the client’s primary care that would help to inform future referrals to the EPI service.

The package includes the following:

    • EPION’s Anatomy of First Episode Psychosis infographic

As we have mentioned when reviewing these at the EPION Network meetings, these documents would be standard across all EPI services; however, we recognize that different clinics may have additional information they provide to primary care as indicated.

        • Generic letter (this letter has general information about the transfer of care)

          • One page information sheet regarding psychosis and what EPI services offer
